This Comprehensive Full-Day Documentation Course offers clinicians a detailed guide to best practices in clinical documentation, striking a balance between meeting insurance and licensing requirements while exercising cautious judgment to safeguard both client well-being and professional integrity. We delve into essential topics such as record keeping, conducting comprehensive psychotherapy intakes, differential diagnosis of common presentations in private practice, treatment plan development, progress notes, and navigating the complexities of commercial and state insurance. Participants will gain insight into what constitutes effective charting to withstand audits or chart reviews, ensuring confidence that their documentation aligns with ethical and professional standards. Given the current political climate in the US, where reproductive rights are increasingly contested, we address the legal and ethical considerations that perinatal mental health providers face post- Roe v. Wade, emphasizing caution in practice for anyone working with folks assigned female at birth (AFAB) of reproductive age [15-49]. Mandated reporting will also be explored.
We will explore Connecticut laws and touch on legislative changes in other states, offering up-to-date information on the evolving legal and political landscape surrounding reproductive rights and the responsibilities of mental health clinicians with consideration for PsyPact & Counseling Compact interacting with CONNIE/Health Information Exchanges.
Objectives:
Identify key components for thorough clinical documentation and construct sample clinical documentation that meets professional and managed care criteria.
Compare mood and anxiety disorders in differential diagnosis exercises.
Create treatment plans with specific considerations to psychosocial circumstances and access to care.
Distinguish goals, objectives, and interventions in treatment planning.
Assess medical necessity for ongoing psychotherapy in case examples.
Evaluate successful discharge criteria & compare how this differs from an administrative discharge
Construct accurate documentation with client’s privacy protection in mind, with consideration of insurance requirements and a medical necessity framework.
Consider current sociopolitical/cultural conditions influence our judicious use of disclosure in the context of complete and accurate documentation
Compose comprehensive informed consent with special attention to understanding of mandates that may influence broader societal safety.
Explore action steps moving forward to continue advocacy efforts as they relate to mental health client privacy, and reproductive well-being.
